FORMS COMPLETION FEE NOTICE
Effective immediately, please be advised that there is a fee for the completion of non-insurance medical forms.
This includes, but is not limited to:
- Disability forms
- FMLA paperwork
- Work/school clearance forms
- DMV or other third-party documentation
- Any letters or forms not directly related to a current medical encounter or covered insurance service
Fee Amount: $25.00 per form (or per packet, as applicable), as well as any recompletion of a form or additional forms of the same type.
This fee reflects the administrative time required by clinical staff to accurately complete and review documentation outside of routine patient care.
Forms will be completed within 7-10 business days after receipt of payment and all required patient information.
Payment is due at the time forms are submitted unless otherwise arranged.
