Patient Info & FAQs

The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit. Please be prepared to provide this information to our office along with your current medical insurance information. If you have any questions when filling out these forms, please do the best you can and our staff will assist you with your questions on the day of your visit.

What do I need for my first visit?

Please bring with you on your first visit:

  • Any documentation from your referring physician
  • Pre-completed patient forms available from our website
  • Your current insurance cards & any necessary pre-authorizatons
  • A photo ID
  • Glasses for both near and far vision or a contact case if required.
  • A complete list of your current medications
  • A list of questions for the doctor
  • Sunglasses
Do I need someone to drive me to my visit?

Both of your eyes will be dilated on your visit. If you are uncomfortable driving in this condition, please arrange for someone to drive you.

How long does a visit normally take?

Please be prepared to be here for at least 2 hours on you first visit. Follow up visits usually take at least 1 hour. This time is needed in order for your eyes to completely dilate. Full dilation is required to efficiently examine and test your condition. The staff will inform you if there is any delay to our schedule.

Where do you perform surgeries?

Any procedures, not able to be performed in our office, are scheduled at either Short Hills Surgery Center in Millburn, St. Barnabas Ambulatory Care Center in Livingston, or Clara Maass Medical Center in Belleville.
